Supplementary Documentation

Depending on the program you have applied to, you may be required to complete and submit supplementary documentation. See below for required and optional supplementary documentation forms.

Required Supplementary Forms

Applicants applying to the following programs must complete additional background information:

Associate Diploma in Turfgrass Management

The Turfgrass Background Information Form is required for all applicants to the Associate Diploma in Turfgrass Management program. Forms and other required documents received after February 15th will be considered, space permitting.

Available: November 15
Deadline: February 15

Bachelor of Landscape Architecture

The BLA Background Information Form (BIF) is required for all applicants to the Bachelor of Landscape Architecture Program.

Available: December
Deadline: May 1 (for Transfer Students)

Doctor of Veterinary Medicine

The DVM Background Information Form required for all applicants to the Doctor of Veterinary Medicine program.

Available: December
Deadline: February 1

Optional Supplementary Documentation Forms

Bachelor of Engineering

Engineering Personal Letter: In order to assist the School of Engineering Admissions Committee in selecting the most qualified applicants, you are encouraged to submit a letter about yourself. The committee is most interested in your background and motivation as it pertains to the study of engineering.

Deadlines:

  • Fall semester - June 1
  • Winter semester - December 1
  • Summer semester - April 1

Bachelor of Applied Science

Applied Science Personal Letter: In order to assist the Bachelor of Applied Science Admission Committee in selecting the most qualified applicants, you are encouraged to submit a letter about yourself. The committee is most interested in your background and motivation as it pertains to the study of applied science.

Applied Human Nutrition:

  • June 1

Adult Development; Child, Youth and Family:  

  • Fall semester - June 1
  • Winter semester - December 1
  • Summer semester - April 1

Personal Support Letter

Transfer applicants are welcome to submit a personal letter to Admission Services detailing any circumstances that may have affected their academic performance positively or negatively.

Deadlines:

  • Fall semester - June 1
  • Winter semester - December 1
  • Summer semester - April 1

Education Declaration

Upon receipt of your application for admission to U of G, you may have been asked to complete the Education Declaration Form. You will know if you are required to complete this form by reviewing either communication received by Admission Services which specifies what supporting documentation you are required to submit or if you see "Education Declaration Form" as a required document on your WebAdvisor account. If so, please complete the above PDF and submit it to applicant@uoguelph.ca.
